Do you know your Street Address? Search our Interactive Map using Block and Parcel or Zoom to a location to find out. You are also able to search by Street Address if you need to find out your Block and Parcel number.
New Requests for naming a New Road shall be submitted using our online Street Name Request Form below and shall include the following:
- The proposed name
- Development name, block/parcel.
- Reason(s) for proposal.
Requests for renaming a road should be submitted on the Street Name Change Form below and shall include the following:
- The present name.
- The proposed name.
- Signatures and address' of 100% of the affected, resident, Registered Proprietors, including strata owners.
- Reason(s) for proposal.
Completed requests should be returned to the Department or emailed to streetaddressing@gov.ky for the Minister of Lands to review
